Compulsory Practice / Internship Form Filling Process


The student, who have received approval from the institution where they want to do practice or internship, receives the Compulsory Practice/Internship Form prepared by the University from the University's web page or from the Applied Education and Internship Coordinatorship. 

The student fills out the Compulsory Practice/Internship Form in 3 copies and have it approved by the place where they will practice/intern in, and then have it approved by the Department/Program, Internship/Vocational Practice Commission Head and Faculty/School Internship/Practice Coordinator. 

​The form filled in 3 copies must be approved by the Applied Education and Internship Coordinator's Office and one copy must be submitted together with 1 copy of ID card, 1 copy of student ID card and 1 photograph. ​

  • All reminders and footnotes on the form must be read.
  • 3 forms are to be filled in, preferably with a blue ballpoint pen (each one handwritten) or on a PC.
  • SPAS (Health Provision Activation System) must be marked on the form. It can be checked and learned via E-Government.
  • One of the forms is submitted to the Applied Education and Internship Coordinatorship and one is submitted to the place of practice/internship. 1 copy should be kept with the student to be evaluated with your documents at the end of the practice/internship.

In accordance with the legal legislation, "Work Accident and Occupational Diseases Insurance" is carried out by our University. If the student is doing practice/internship in more than one practice/internship place, they can use the insurance certificate made until the end of the practice/internship in the last practice/internship place.
